Free printable caregiver daily checklist template

One page a day. Medications, meals, symptoms, sleep, and a single line for how you are doing. Print it, clip it to the fridge, and bring the week to the next appointment.

What's on the checklist

The template splits the day into three short passes so nothing depends on remembering at 10pm.

Morning

  • Medications given (name, dose, time)
  • Breakfast and fluids
  • Bathing, dressing, oral care
  • Mood and alertness check
  • Blood pressure / blood sugar (if tracked)

Afternoon

  • Lunch and fluids
  • Movement, walk, or physical therapy exercises
  • Appointments, calls, or visitors
  • Pain level (0–10) and what helped
  • Toileting / continence notes

Evening

  • Dinner and evening medications
  • Skin check and repositioning
  • Sleep prep and bedtime routine
  • Anything unusual to tell the doctor
  • One line for the caregiver: how am I doing?

Weekly rollup

  • Missed or late medications
  • Pain and mood trend
  • Sleep quality
  • Questions for the care team

How to use it

  1. 1Print one page per day, or a week at a time, on standard US Letter paper.
  2. 2Fill it in as care happens — in the moment beats reconstructing the day later.
  3. 3Have every caregiver initial their entries so a rotating team stays in sync.
  4. 4Bring the last seven days to appointments; patterns speak louder than memory.

Questions caregivers ask

What should a caregiver daily checklist include?

At minimum: medications with times, meals and fluids, personal care, mobility, pain or symptoms, sleep, and a short notes line for anything unusual. Grouping the day into morning, afternoon, and evening keeps it fast to fill in.

Is this caregiver daily checklist template free to print?

Yes. The Daily Care Log inside the LightWell Care Starter Kit is free to download and print at home on standard US Letter paper — no subscription required.

How do I use a daily checklist at a doctor's appointment?

Bring the last seven days. Patterns — missed doses, rising pain scores, poor sleep — are far more useful to a clinician than memory, and they shorten the visit.

Should more than one caregiver use the same checklist?

Yes. A shared checklist prevents double-dosing and missed care when family members or paid aides rotate. Each person initials what they completed.
