Call 911 if you notice
- Chest pain or pressure
- Difficulty breathing
- Sudden confusion
- Signs of stroke (face drooping, arm weakness, speech difficulty)
- Severe or uncontrolled bleeding
- Loss of consciousness
Bring to the hospital
- Current medication list
- Insurance card
- Photo ID
- Advance directive / POLST
- This care binder
Questions to ask right away
- 1.What is the diagnosis or suspected cause?
- 2.What tests or treatments are being done?
- 3.What are the next steps?
- 4.How long will they stay?
- 5.Who is the attending doctor?
